About the Role
As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for our participants. Your mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, in the right way, while encouraging engagement with our virtual care platform.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether facilitating a self-led journey or managing a traditional referral, your focus remains on delivering a friction-free, positive experience that empowers individuals to improve their quality of life.

Key Responsibilities

- check_circleProcess service referrals via email, text, web-form, and telephone
- check_circleConduct triage and assessment of participant needs and goals
- check_circleGuide participants through onboarding and intervention selection
- check_circleManage participant records and capture minimum data sets
- check_circleEnsure compliance with data protection, safety, and safeguarding policies
Requirements
- verifiedProven track record in customer-facing role (face-to-face and telephone)
- verifiedExperience with Case Management Systems
- verifiedExperience supporting vulnerable individuals through change
- verifiedExperience with Microsoft Office
- verifiedExperience in data input/processing with accuracy and deadlines
- verifiedExperience in health, community, or social care environment
- verifiedUnderstanding of social determinants of health
- verifiedUnderstanding of population-based approaches (segmentation, risk stratification)
- verifiedUnderstanding of behaviour change principles
- verifiedSkills in motivational interviewing, conversation frames, and patient activation
- verifiedCommitment to flexible working (evenings and weekends)
Nice to Have
Relevant health coaching qualification or an accredited health coaching skills programme. Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard. Seeks self-improvement.
