About the Role
As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through the triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will support participants in accessing the right intervention at the right time and in the right way, while encouraging them to utilize our virtual care platform for a self-led journey.
In this role, you will facilitate informed choices regarding care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through a professional service delivery or compassionate communication, your focus is on creating friction-free referral pathways that support a positive participant experience. You will manage a proactive caseload, engaging with individuals via telephone, text, email, and our virtual platform to drive successful onboarding to interventions.

Requirements
- verifiedProven track record in customer facing role (face to face and telephone)
- verifiedExperience with Case Management System
- verifiedExperience supporting vulnerable individuals through change process

- verifiedExperience in health, community or social care environment
- verifiedStrong understanding of social determinants of health
- verifiedStrong understanding of population-based approaches

Nice to Have
Relevant health coaching qualification or an accredited health coaching skills programme. Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard. Seeks self-improvement.
