The Referral Coordinator serves as a key liaison between patients, providers, internal care teams, and external partners. This role is critical to supporting a centralized referral management model that enhances patient experience, ensures continuity of care, and drives operational efficiency. You will ensure that all internal and external patient referrals are managed, coordinated, and processed to provide timely and seamless access to care.
In this position, you will act as the primary point of contact for navigating referral-related requests. Your work ensures that referrals are tracked, documented, and closed appropriately within the Electronic Health Record (EHR), strictly aligning with organizational standards and payer requirements. By managing the full life cycle of referrals through a closed-loop process, you will help identify trends and gaps to continuously improve service delivery across our service lines.
