As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for participants entering our service. Your primary mission is to guide individuals through the triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, in the right way, while encouraging engagement with our virtual care platform.
This position is designed to support participants in making informed choices about their care pathways. You will explore a variety of behaviour change options, including in-house services, sub-contracted providers, and adjacent service options, ensuring a seamless and positive experience for every individual.
Your journey begins by processing referrals received through multiple channels, including email, text, web forms, and telephone. You will provide professional service and compassionate communication, conducting effective triage to assess needs and goals. A key part of your day involves managing the referral inbox and facilitating timely onboarding, whether that means inviting participants to the virtual care platform for a self-led journey or creating records for those who require direct support.
You will also handle sensitive data capture, including minimum data sets, consent forms, and baseline questionnaires. This includes identifying accessibility considerations and safeguarding concerns with the utmost care. For those unassigned to an intervention, you will proactively manage your caseload through responsive engagement via phone, text, email, and the virtual platform. Additionally, you will ensure all activities are accurately captured to drive successful onboarding and maintain dashboard visibility on referral numbers and participant journeys.
