About the Role
Element Care is seeking a Project Manager, Clinical and Operational Innovation to lead cross-functional initiatives that enhance care delivery for PACE participants. This role exists to drive the evolution of how we serve our community by improving clinical workflows, operational efficiency, and patient outcomes.
In this position, you will partner closely with clinicians, operations teams, IT, and leadership to define project scope, timelines, and success metrics. Your day-to-day involves managing risks and resources while ensuring all initiatives align with regulatory standards and Element Care's core mission. You will leverage data to drive decisions, facilitate change management, and foster a culture of continuous improvement, collaboration, and compassionate senior care.
