Longevity Scripts is seeking an on-call Pharmacist for our Fort Myers location. This role is vital to ensuring the safety and efficacy of medication therapy for our residents, bridging the gap between clinical expertise and daily operational needs.
As an on-call Pharmacist, you will evaluate physician medication orders for appropriateness, dosage, potential interactions, route of administration, and stability. You will serve as a clinical resource, making recommendations for therapy changes to physicians and facility staff while conducting Drug Utilization Reviews (DUR) on all prescriptions to prevent adverse drug interactions. Your day-to-day responsibilities include maintaining comprehensive patient databases with current and past medical history, clinical data, and concurrent medications, as well as collaborating with pharmacy technicians to reconcile controlled substance logs with inventory to ensure compliance with state and federal regulations.
In this dynamic environment, you will provide clinical expertise by responding to drug-related questions from internal teams (technicians, nursing staff) and external stakeholders (Directors of Nursing, administrators). Clear documentation of all communications with the healthcare team and patients' families in the medical record is essential. As an on-call staff member, you must be available to work at least one shift per month, with schedules that may include rotating weekends and nights.
