As a Practice Transformation Specialist (PTS), you will serve as the primary point of contact between Aledade HQ and our partner practices. Your mission is to make a meaningful impact on patient care by developing valuable insights into providers' needs and satisfaction. You will be passionate about population health and committed to delivering better, more cost-effective care. This role offers the opportunity to strengthen your community's health by transforming practices one at a time.
