About the Role
As a Practice Transformation Specialist (PTS), you will serve as the primary point of contact between Aledade HQ and our partner practices. Your mission is to make a meaningful impact on patient care by developing valuable insights into providers' needs and satisfaction. You will be passionate about population health and committed to delivering better, more cost-effective care. This role offers the opportunity to strengthen your community's health by transforming practices one at a time.

Key Responsibilities
- check_circleImplement products and execute ACO initiatives under Market Leadership direction
- check_circleSupport wellness visits, transitions of care, and active management to meet practice goals
- check_circleTrain office staff and physicians on processes and technologies using the Value Based Care Curriculum
- check_circleDevelop relationships with physicians and executives to enhance financial and quality performance
- check_circleTravel up to 40% of the year to visit practices onsite for direct support
- check_circleListen to practice concerns and relay critical information to management for improvement opportunities
- check_circleAssess practice and staff training needs to ensure continuous improvement
Requirements
- verifiedMust be resident of Florida
- verifiedGraduate of an accredited college or university OR 4-6 years of significant work experience in medical practice management
- verified3 years of experience with Electronic Health Records (EHR)
Nice to Have
Excellent oral and written communication skills, including public presentation skills; organizational and project management skills; team-building skills; familiarity with process mapping and work-flow analysis tools; excellent computer skills and willingness to learn additional software applications.
