The Practice Transformation Coach serves as a strategic advisor and subject matter expert within Astrana's value-based care network, driving clinical quality improvement and operational performance across provider practices in the New York metropolitan area. Operating with a high degree of autonomy, you will partner closely with physicians, practice leadership, and care teams to analyze practice-level performance, identify gaps, and develop targeted, data-driven strategies that improve quality outcomes, financial performance, and operational efficiency.
In this critical role, you will align provider performance with value-based care objectives, leading workflow redesign and population health initiatives within a complex, diverse urban healthcare environment. You will act as a key liaison between provider practices and organizational leadership, influencing decision-making through data-driven insights and consultative engagement to drive measurable improvements across the market.
Your work will be a dynamic blend of field-based coaching and strategic analysis. Approximately 50–70% of your time will be spent traveling across the five boroughs and surrounding downstate regions to conduct onsite coaching, workflow assessments, and performance improvement initiatives. You will assess practice needs, prioritize interventions, and lead the implementation of customized transformation plans to optimize EHR usage, clinical documentation, and billing practices.
When not onsite, you will focus on remote analysis of complex performance data, including HEDIS and pay-for-performance metrics, to inform strategic recommendations. You will collaborate with internal stakeholders across Market Operations, Quality, and Population Health to ensure alignment on initiatives, while also contributing to the development of best practices and frameworks that enhance the broader network's transformation efforts.
