About the Role
As a Physician Assistant on our team, you will play a pivotal role in a collaborative, interdisciplinary environment. You will partner closely with physicians, nurses, and care team members to provide comprehensive primary care to Medicare-eligible seniors. This is not just a clinical position; it is a mission-driven role where you will leverage data-driven tools and EMR systems to close care gaps and manage chronic diseases effectively.
In this team-based capacity, you will perform comprehensive assessments, manage acute and chronic issues, and coordinate care to ensure seamless patient experiences. Your day will involve conducting history and physical exams, developing evidence-based treatment plans, and educating patients and families on their conditions and medications. You will also participate in quality improvement initiatives and team huddles to continuously enhance the standard of care we deliver.
