About the Role

This is a flexible, meaningful part-time opportunity for a board-certified Nurse Practitioner to conduct In-Home Health Evaluations for Medicare Advantage, Medicaid, and commercial plan members. The role is designed for those with a passion for engaging with people and helping them on their journey to better health.

In this position, you will travel locally within a designated metro area (primarily Springfield, MA) to conduct pre-scheduled visits. During these approximately 45-minute interactions, you will meet members in their homes to review medication and medical history, perform physical evaluations, and conduct additional diagnostic tests if ordered by the health plan. You will be provided with pre-populated, patient-specific forms to guide these evaluations. While you will not be required to prescribe medicine, order lab tests, or alter current treatment regimens, you will have the time to truly listen to questions, provide education, and help guide members toward better self-care.

The role emphasizes closing gaps in care and keeping people safe and well at home. You will have the option to provide availability Monday through Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m., allowing you to balance your schedule while making a tangible impact in your community.
