We bring together Physical Therapists, nurses, occupational therapists, and social workers to coordinate comprehensive services that support patient independence and safety.
As a Physical Therapy Assistant, you will play a vital role in executing treatment plans developed by licensed Physical Therapists. Your day will involve traveling to various home environments to provide physical therapy interventions, including therapeutic exercises, gait and balance training, and functional mobility activities. You will be the eyes and ears of the care team, monitoring patient responses, documenting findings accurately in the electronic medical record, and promptly reporting any changes in status.
Beyond clinical execution, you will serve as an educator, instructing patients and caregivers in home exercise programs, safe mobility techniques, and fall prevention strategies. Success in this role requires managing your daily routes efficiently while ensuring patient safety in diverse home settings. You will actively participate in case conferences and continuing education, upholding VNS Health's core values in every interaction.
