About the Role
We are seeking licensed Physical Therapists and Physical Therapist Assistants who are passionate about making a difference in the lives of our patients. As a Per Diem provider, you will enjoy the flexibility to choose your own patients and schedule. Your day will involve conducting in-home physical therapy visits, performing comprehensive evaluations and reassessments, and executing fall risk assessments. You will play a vital role in documenting patient progress and communicating effectively with the broader care team to ensure cohesive, patient-centered care.
