About the Role
As a Physical Therapist at our Delta, Colorado skilled nursing community, you will lead the delivery of individualized care programs designed to maximize patient progress toward functional goals. Your day will involve assessing patient needs, planning and implementing treatment strategies, and providing essential education to patients and their families. You will serve as a vital resource within the care team, collaborating closely with other health professionals and ancillary staff to ensure optimal outcomes.
In this role, you will direct the assistance of support staff to achieve patient goals, conduct therapy practice within defined standards of care, and actively participate in quality improvement and safety initiatives. You will also foster professional growth by engaging in educational programs and staying current with professional literature, while acting as a leader and advocate for physical therapy within the community.
