About the Role
As a Physical Therapist Assistant in Home Health, you will provide patient-centered physical therapy services under the supervision of a licensed Physical Therapist. Your mission is to implement individualized plans of care designed to improve mobility, restore function, and prevent disability.
In this role, you will work directly with patients to assist with therapeutic exercises, wound care, and modalities while ensuring all treatment is documented accurately in the EMR. You will practice in accordance with state regulations, professional standards, and CCH's mission and values, demonstrating fiscal responsibility, accountability, and strong time management skills. Flexibility is key, as you may need to provide coverage to other service lines or adjust to varying census levels to meet patient needs.
