Key Responsibilities
- check_circleProvide physical therapy interventions for seniors in skilled nursing and long-term care settings
- check_circleImplement individualized treatment plans to improve mobility, function, and safety
- check_circleDocument patient progress, outcomes, and treatment in accordance with regulatory standards
- check_circleEducate patients, families, and caregivers on exercises, safety, and functional strategies
- check_circleMaintain compliance with facility, state, and federal regulations and clinical quality standards
- check_circleCollaborate with interdisciplinary teams to adjust care plans and support discharge planning
- check_circlePerform therapeutic exercises, gait training, and transfer and mobility training
- check_circleConduct patient assessments and utilize EMR systems for documentation and care planning
Requirements
- verifiedCurrent state-licensed Physical Therapist Assistant (PTA)
- verifiedGraduation from an accredited PTA program
- verifiedBasic Life Support (BLS) or CPR certification
- verifiedExperience or training in geriatric rehabilitation preferred
