We are seeking a compassionate Physical Therapist to join our team and deliver high-quality rehabilitation services. In this role, you will evaluate and treat patients with musculoskeletal, neurological, and cardiopulmonary conditions, developing individualized treatment plans that improve mobility, strength, balance, and function.
You will work closely with an interdisciplinary team of physicians, nurses, and other rehabilitation professionals to coordinate care plans and ensure safe transitions during discharge planning. A key part of your day involves educating patients and families on home exercise programs and safe mobility techniques, while maintaining accurate documentation in our EMR system. You will also actively participate in quality improvement and safety initiatives to uphold best-practice standards.
Qualified candidates should submit their application including their current state Physical Therapist license, Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program, and Basic Life Support (BLS) certification. Experience with EMR systems such as Epic is required. Please note that ACLS certification is preferred depending on the specific unit, and specialty certifications (e.g., OCS, NCS) are a plus.
