In this position, you will serve as a vital member of the care team, collaborating closely with physicians, nurses, and other healthcare professionals to coordinate comprehensive, patient-centered care. Your primary mission is to evaluate and treat patients with a variety of conditions, developing and implementing individualized, evidence-based plans of care to improve mobility, strength, balance, endurance, and overall functional independence.
Beyond clinical interventions, you will play a key educational role by teaching patients and their families about diagnosis, prognosis, safety strategies, and home exercise programs to promote recovery and prevent complications. You will also participate in discharge planning, making recommendations for continued therapy and necessary equipment to ensure a smooth transition of care.
