As a Physical Therapist with VNA, you will serve as a vital member of our interdisciplinary team, dedicated to delivering compassionate, high-quality home health care. This role exists to ensure that patients receive skilled, evidence-based therapy in the comfort of their own homes, helping them regain independence and improve their quality of life.
In this position, you will be responsible for evaluating patient needs, developing comprehensive plans of care, and coordinating with physicians and other care providers to address significant changes in patient conditions. Your day-to-day involves conducting skilled evaluations, providing direct care and instruction, and supervising Home Health Aides to ensure continuity of care. You will also play a key role in maintaining national standards for home health care by utilizing HHRG values to establish visit frequencies and ensuring annual outcomes meet national benchmarks.
Beyond clinical duties, you will be the face of VNA's commitment to CARES—demonstrating compassion, accountability, respect, excellence, and service in every interaction with patients, families, visitors, and colleagues. Your clear and concise documentation will reflect the specifics of teaching, direct care, and patient responses, ensuring a seamless record of skilled care delivery.
