This field-based position serves as a critical link in the continuum of care for Personal Care Assistance (PCA) members. As a Contractor-PCA RN Assessor, you will perform in-home assessments to determine the appropriate level of care and services authorized for patients. Your work ensures that clinical needs are evaluated against established guidelines, guaranteeing that services are medically appropriate and aligned with the benefit structure.
You will act as an advocate for members and their families, coordinating with patients, families, physicians, hospitals, and other external stakeholders to manage care from diagnosis through outcome. The role requires a balance of clinical expertise and fiscal accountability, evaluating the necessity, appropriateness, and efficiency of services while participating in quality improvement programs to enhance member outcomes.
Your day involves traveling to patient homes to conduct comprehensive assessments, documenting findings accurately based on standards of practice and organizational policies. You will interact with multidisciplinary teams—both telephonically and in person—to ensure continuity of care, occasionally providing testimony for Medical Fair Hearings. A key metric of this role is completing a minimum of five PCA assessments daily, requiring strong time management and organizational skills to navigate a busy schedule of field visits and coordination efforts.
Interested candidates must possess an active, unrestricted New Jersey RN License. While a Bachelor's degree is preferred, relevant experience may be considered in lieu of the degree. Please ensure you have a valid New Jersey State Driver's License and insurance, as daily travel to patient homes is required for this field-based position.
