This position serves as a vital bridge between referring physicians, patients, and caregivers, ensuring seamless access to clinical and supportive care services both within the hospital and the broader community. As a dedicated point of contact, you will facilitate patient appointments across labs, diagnostic areas, and with specialty physicians to maintain a robust continuum of care.
The role is centered on coordinating specialized diagnostic breast imaging and managing the care journey for patients facing breast disease. You will apply your clinical expertise to assess patients' physical and psychosocial statuses, developing and implementing comprehensive care plans that address the unique challenges of breast cancer treatments and recovery. Beyond clinical management, you will play a key educational role by developing and delivering in-house professional development programs and community outreach workshops.
You will work collaboratively within a multidisciplinary care team, coordinating activities with University Medical Center departments, county agencies, and various public and private medical providers. Success in this role requires the ability to communicate openly and empathetically with diverse audiences, including patients, families, professional staff, and the media. You will be expected to demonstrate flexibility in responding to changing system needs while maintaining a high standard of patient-centered care.
