We are seeking an experienced Home Health RN Case Manager to join our team in Dallas, TX, and support our growing geriatric population. This role is designed for a nurse who thrives in a people-first culture where you can make a tangible difference in the lives of those in need. You will provide comfort, dignity, and support while working alongside a caring and collaborative group of professionals.
In this position, you will lead clinical nursing and aide personnel, ensuring the quality and continuity of services. Your day will involve partnering with patients, representatives, and caregivers to develop and evaluate comprehensive plans of care. You will coordinate services with physicians and other healthcare practitioners, integrating orders and monitoring patient conditions to ensure regulatory compliance. Beyond clinical duties, you will serve as an educator and counselor, providing guidance to patients and families while fostering a culture of professional growth and competency within the team.
