About the Role
The Patient Continuity & Needs Advocate is the heartbeat of our patient experience, responsible for cultivating positive interactions and driving long-term member retention. This role exists to ensure seniors feel supported, heard, and connected to the care they need.
In this position, you will work collaboratively with Membership Growth Consultants and Center Dyads to execute targeted activities that keep our members engaged. You will act as a lobby ambassador, identifying opportunities for referrals and service recovery while building strong relationships with brokers and agents. Your day will involve leveraging patient experience tools to proactively identify members for early intervention and connecting disenrolled members with the right resources to reenroll. You will be a trusted partner to our patients, helping them maximize their medical benefits and navigate the complexities of healthcare with confidence.
