As a Patient Care Coordinator, you will become a vital part of our caring community. In this role, you will serve as a bridge between our clinical teams and key external partners, including hospital discharge planning services, skilled nursing facilities, and assisted living management. Your primary focus will be to maintain ongoing contact with these facilities to provide updates on our services and to conduct facility visits at the request of physicians to assist the clinical team in determining patient eligibility.
You will ensure effective communication and collaboration with program staff and field sales resources through active participation in weekly meetings. Additionally, you will assist the program in processing physician orders as directed, reporting directly to a Market Executive of Sales.
