We are seeking an experienced Part-time MSW or LICSW to join our home health team in King County, covering areas such as Kent, Burien, and Renton. In this role, you will serve as a vital liaison between patients and community agencies, assessing the psychosocial status of patients related to their illness. You will develop and provide multiple community and governmental resources to support patients in their home environments, carrying out social evaluations and planning interventions. Your work will involve participating in the development of individualized plans of care and assisting physicians in understanding significant social and emotional factors. Additionally, you will contribute to quality assessment and performance improvement teams while maintaining and developing contracts with public and private agencies.
As a PRN employee, you will receive competitive compensation of $38-$45 per hour (DOE), along with mileage reimbursement, flexible scheduling, and access to an Employee Assistance Program and Employee Emergency Funds.
