This position is part of the PACE (Program of All-Inclusive Care for the Elderly) program, dedicated to serving a medically complex geriatric population within an outpatient and community-based setting. The role operates within a collaborative interdisciplinary care model focused on preventive care, chronic disease management, and significantly improving the quality of life for elderly participants.
As a provider, you will manage a daily patient volume of 8 to 12 individuals. The position requires a flexible approach to care, including making 1 to 2 calls per week to support patients outside of direct clinical hours. The team includes Medical Assistants (MAs) and additional support staff to facilitate comprehensive care delivery.
The role is based in California and utilizes the Epic EMR system. The standard shift is Monday through Friday, 8:00 AM to 5:00 PM (5x8 schedule). A one-hour daily block is allocated specifically for chart completion. During the first week, orientation will follow a 5x8 schedule regardless of the regular shift and may involve rotations across multiple sites.
This is a 3-month commitment with an option to extend. Housing and travel expenses are fully inclusive. Credentialing is typically completed within 2 to 3 weeks after accepting the assignment, with a start date of ASAP.
Candidates must hold an active California license, BLS certification, and DEA registration. Board Certification (BC) or Board Eligibility (BE) is required. The position is open to physicians who have recently completed residency. Any missing skills, procedures, or limitations regarding patient populations must be disclosed prior to presentation.
