This position is uniquely designed for individuals with lived experience or professional expertise in HIV, Hepatitis C, HIV Pre-exposure Prophylaxis (PrEP), or substance use disorder. The core mission is to enhance the quality of life for patients by providing comprehensive support, including healthcare coordination, education, and linkage to essential services.
In this role, you will assess the needs of at-risk patients to facilitate testing, linkage, and referrals. You will work collaboratively with healthcare providers, pharmacies, insurance companies, social service agencies, and community organizations to ensure patients receive timely, appropriate, and holistic care. The Outreach Navigator plays a vital part in outreach activities, driving patient recruitment and screening efforts to expand access to care.
This position operates within the context of Maricopa County, focusing specifically on the cultural and social characteristics of Native American and ethnic minority populations.
