About the Role
We are seeking a detail-oriented and proactive professional to join our Enrollment Team and support our Clinical Team. This full-time, onsite position is based in Jacksonville, FL, with a schedule that rotates between two local practice locations.
In this multifaceted role, you will work directly alongside providers to enroll patients in Care Management programs. Your day will involve conducting conversations with eligible patients while they are in the office, clearly explaining the benefits of our care programs for managing chronic conditions and improving overall health. You will serve as a bridge between the clinical team and patients, verifying eligibility, maintaining accurate records, and communicating with empathy to build trust and encourage participation. Success in this role requires strong organizational skills, the ability to manage multiple priorities in a busy clinical environment, and a commitment to supporting the integrity of patient charts.
