Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
This position is dedicated to providing compassionate, on-call patient visits for urgent physical and psycho-social issues during after-hours shifts. Working in coordination with the Call Center Triage and On Call Manager, you will be the vital link between the patient, their family, and the broader care team when needs arise outside of standard business hours.
You will be available via cell phone to respond to triage visit requests, communicating effectively with the on-call team, including the Hospice physician, to address urgent patient needs and establish a safe plan of care. Your day involves implementing these plans, educating patients and primary caregivers on changes regarding medication management, patient safety, and symptom management. Additionally, you will attend patient deaths as directed to provide essential emotional support and guidance to grieving families.
