About the Role
As a Home Health Occupational Therapist with SSM Health at Home in Mount Vernon, you will serve as a vital link in the continuum of care for patients recovering in the comfort of their own homes. This PRN role is designed for professionals who are passionate about helping individuals regain independence and improve their quality of life.
In this position, you will evaluate and treat patients facing physical, emotional, cognitive, and psychosocial challenges. Your day will involve assessing a patient's ability to perform independent living skills safely and effectively, then designing a personalized plan of care that addresses their specific deficits. You will implement treatment plans using appropriate therapeutic modalities, document progress, and continuously monitor and modify care strategies as needed.
Beyond clinical treatment, you will play a key educational role, guiding patients and their families or caregivers through the recovery process. This includes planning for discharge, recommending adaptive equipment, and connecting patients with support programs. You will also oversee the care provided by designated individuals to ensure it meets established regulations and practice standards, all while maintaining a constant state of alertness and safety.
Hiring Process
Candidates for this role must hold a Bachelor's, Master's, or Doctorate degree in Occupational Therapy or a related field, along with the required state licensure. While no prior experience is required, successful applicants must possess valid professional certifications, including Basic Life Support (BLS) and a valid driver's license with an acceptable driving record, specific to the state of work (Illinois, Missouri, Oklahoma, or Wisconsin).
