Clarion Wellness & Rehabilitation in Clarion, IA, is a skilled nursing facility providing long-term care, post-acute rehabilitation, and geriatric outpatient therapy services. Unlike facilities that rely on external contract agencies, we maintain a dedicated in-house therapy team. All department members work directly for the facility, sharing unified goals and benefiting from job stability within a growing company.
Our organization operates with a unique model where each facility is independently managed by local leadership. This structure eliminates corporate red tape, ensuring that decisions are made specifically for the facility, its staff, and the community needs. We are united by our core values (CAPLICO): Customer Second, Accountability, Passion for Learning, Love One Another, Intelligent Risk Taking, Celebration, and Ownership.
As an Occupational Therapist, you will join a supportive environment designed to offer work-life balance through a flexible PRN schedule. This role is ideal for therapists seeking to complement full-time employment or those looking for a set schedule to cover vacations. You will be part of a close-knit team where you can participate in CEU classes offered to the entire therapy group.
In this position, you will effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders, developing appropriate plans of care that adhere to all regulatory and clinical practice standards. You will utilize standardized assessments and evidence-based practices to drive clinical interventions, administering procedures such as ADL retraining, therapeutic activities, exercises, and the use of assistive devices.
Your day will involve assessing patient progress, adjusting treatments in the clinical record, and evaluating treatment outcomes to modify services as needed. You will also provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Strong communication with your supervisor and the broader health team is essential to discuss patient progress, barriers, and treatment plans, culminating in comprehensive discharge summaries.
