This Per Diem Nurse Practitioner position covers all zip codes in New York County, New York. The role offers a unique opportunity to spend a full hour with each member, allowing you to capture a comprehensive picture of their personal needs. Your work is critical in determining what is required to keep members in the home and out of the hospital.
You will practice autonomously in a remote clinical environment, independently conducting patient assessments, formulating evidence-based treatment plans, and managing complex chronic conditions. The position involves making sound clinical decisions without direct on-site supervision, addressing gaps in care, and supporting the physician-patient relationship through timely communication and documentation.
Candidates must be available for a minimum of 25 slots per month (approximately 37 hours). The role requires a full hour per assessment to ensure thorough care. Please note that earnings vary based on completed assessments, state of residence, and business needs, with no guarantee of visits or minimum income.
