This Per Diem Nurse Practitioner position covers all zip codes in Lancaster County, Nebraska. The role offers a unique opportunity to spend a full hour with each member, allowing you to capture a comprehensive picture of their personal needs. This depth of interaction is essential for determining what is required to keep members in the home and out of the hospital.
You will practice autonomously in a remote clinical environment, independently conducting patient assessments, formulating evidence-based treatment plans, and managing complex chronic conditions. Your work will involve making general recommendations to improve members' knowledge of their chronic conditions, addressing gaps in care, and recognizing emergent situations requiring escalation. By supporting the physician-patient relationship and ensuring timely communication with primary care providers and health plans, you will play a critical role in the continuity of care.
Candidates must possess an active un-encumbered license to practice nursing and board certification as a Nurse Practitioner or Clinical Nurse Specialist. Prior to hire, compliance with recommended Healthcare Personnel Requirements for vaccinations and preventive testing is mandatory. The role requires a minimum availability of 25 slots per month (approximately 37 hours).
