HarmonyCares is a mission-driven, patient-first organization dedicated to providing compassionate home-based primary and palliative care to medically complex, home-limited patients. We operate within a rapidly evolving healthcare landscape, fostering a culture that values teamwork, respect, and integrity. Our goal is to improve patient outcomes and reduce avoidable hospitalizations by delivering high-quality care directly where patients live.
We are seeking a compassionate Nurse Practitioner to join our interdisciplinary care team. In this field-based role, you will serve as a vital link for patients requiring home health, palliative, and hospice-focused care. Your day will involve conducting comprehensive in-home visits, performing detailed assessments, and managing acute and chronic medical conditions. You will collaborate closely with physicians, nurses, social workers, and therapists to develop and update individualized care plans that address the unique needs of each patient.
Beyond clinical management, you will play a key educational role, guiding patients and family caregivers on disease processes, medication management, and home safety strategies. You will leverage telehealth resources and maintain accurate clinical documentation in our EMR systems. This position offers a flexible schedule, allowing you to prioritize and organize daily home visits within your assigned territory while driving meaningful change in the lives of those we serve.
Interested candidates are invited to submit their application to begin the review process. Qualified applicants will be contacted for an initial screening to discuss their background and experience in home health or palliative care. Successful candidates will then proceed to a clinical interview and a final team fit discussion before an offer is extended.
HarmonyCares is committed to building a diverse and inclusive workforce. We consider qualified applicants regardless of background, race, color, religion, gender, sexual orientation, age, disability, or veteran status. We believe that a diverse team creates a better environment for our patients and our community.
