About the Role

We are seeking a compassionate Nurse Practitioner to join our interdisciplinary care team. In this field-based role, you will serve as a vital link for patients requiring home health, palliative, and hospice-focused care. Your day will involve conducting comprehensive in-home visits, performing detailed assessments, and managing acute and chronic medical conditions. You will collaborate closely with physicians, nurses, social workers, and therapists to develop and update individualized care plans that address the unique needs of each patient.

Beyond clinical management, you will play a key educational role, guiding patients and family caregivers on disease processes, medication management, and home safety strategies. You will leverage telehealth resources and maintain accurate clinical documentation in our EMR systems. This position offers a flexible schedule, allowing you to prioritize and organize daily home visits within your assigned territory while driving meaningful change in the lives of those we serve.

Hiring Process

Key Responsibilities

  • check_circleDevelop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • check_circleManage acute and chronic medical conditions, including prescribing and adjusting medications within NP scope
  • check_circleProvide home health, palliative, and hospice-focused care including symptom management and end-of-life support
  • check_circleDocument all visits, assessments, interventions, and care plans accurately and promptly in the EMR
  • check_circleCoordinate care with physicians, nurses, social workers, and therapists to ensure continuity and quality
  • check_circleMonitor changes in patient condition and adjust care plans or escalate care as needed
  • check_circleAdhere to all regulatory, legal, and organizational standards related to home health and hospice services
  • check_circleMaintain infection control practices and safety standards in patient homes and during travel

Requirements
