The Nurse Navigator is a leader dedicated to providing support, mentorship, and guidance to those who deliver direct patient care. This role is designed to facilitate an effective new care model, ensuring patients receive timely, safe, effective, and high-quality care.
You will coordinate the delivery of care by assessing individual patient requirements and making recommendations based on the severity of illness, intensity of service, and skill level. A key part of your day involves collaborating with bedside nurses to coordinate the patient's plan of care, holding providers accountable for their professional responsibilities and adherence to regulatory guidelines. You will manage both inpatient and outpatient populations, focusing on quality patient care, safety, and financial components.
Your impact will be felt through pre-op education, daily rounding, managing discharge dispositions, and coordinating follow-up care. You will also drive length of stay management, utilization management, and the adoption of best practices to optimize performance. This position requires you to perform duties consistent with Mercy's mission, values, and service standards.
