Nurse Case Manager (RN)

As a Nurse Case Manager, you will serve as the central point of contact between patients, families, and healthcare providers. Your mission is to ensure seamless care coordination by conducting comprehensive assessments of patient medical history, medications, treatment plans, and psychosocial needs. You will collaborate closely with the healthcare team to develop, implement, and adjust individualized care plans, monitoring patient progress and maintaining up-to-date records in compliance with all confidentiality standards.

This position offers flexibility with day shift options available in full-time, part-time, or per diem capacities.
