About the Role
The Mosaic at Home Coordinator serves as a vital link for individuals and families living independently, ensuring they receive seamless, person-centered support. This role exists to enhance community health and foster independence by coordinating a blend of in-home and community-based services. You will act as a dedicated advocate, conducting comprehensive assessments and crafting personalized care plans that connect clients to essential medical, social, and community resources.
In this position, you will collaborate closely with clinical teams and external community partners to ensure continuity of care. Your day-to-day involves monitoring client progress, providing education and advocacy, and maintaining accurate documentation within Electronic Health Records (EHR) systems. By participating in case conferences and quality improvement efforts, you will help drive the program's success while addressing barriers to care for those you serve.
