Camden Case Management (CCM) provides management services to individuals facing mental health and addiction challenges, supporting their journey toward successful reintegration. Founded on principles of excellence, compassionate care, and respect, our team collaborates with clients at various stages of their healing process to develop flexible, creative, and individualized reintegration plans.
We operate as a multidisciplinary adjunct to clients' ongoing clinical work with outside providers. While we do not perform clinical services, we support skill building, internal resource allocation, and emotional regulation learned in clinical settings. Our approach emphasizes that regular communication is essential for success, utilizing a comprehensive support structure to help clients move toward functional independence through achievable goals.
As a Mentor in San Jose, you will serve as the client's primary source of support. This multifaceted role requires you to take the lead in many aspects of client care, including daily communication, observation, and reporting. The client will spend the vast majority of their face-to-face hours with you, making your relationship central to their progress.
In this position, you will work independently with minimal supervision while maintaining a high level of communication and organization. You will report directly to a Case Manager, providing them with in-depth and timely updates on the client's status. Your day-to-day involves accompanying clients to recovery-related activities, assisting with life skills such as budgeting and resume building, and ensuring safety and medication adherence. This role is ideal for someone who can remain empathetic and professional while navigating stressful situations or crises.

We encourage team members to maintain other gainful employment to accommodate potential reductions in hours. A valid driver's license and clean driving record are required to conduct home visits and travel to client locations.

Valid driver's license and clean driving record. Openness to pursuing education in the field of mental health, counseling, addiction, or coaching studies. 1 year of relevant experience. Bachelor's degree or higher.
