As a Social Worker within our Clinical Resource Management department, you will serve as a vital link between patients, their families, and the broader healthcare system. Based in Poway, California, this full-time day shift position is designed for those passionate about supporting individuals and families in crisis. You will operate within the social work model to assess needs, identify appropriate resources, and coordinate comprehensive care plans for patients across the Emergency Department, Trauma, and hospital settings.
Your day will involve collaborating closely with physicians and interdisciplinary staff to manage admissions, transfers, treatment, and discharge plans. You will evaluate the transition from acute emergency care to the next level of the healthcare continuum, developing targeted interventions when necessary. By maintaining a deep knowledge of community resources, you will ensure that patients and families have seamless access to the support services they require.
