About the Role
We are seeking a dedicated MDS Nurse to join our team at the Village at Victory Lakes in Lindenhurst, IL. In this PRN (as-needed) capacity, you will play a pivotal role in organizing, completing, and evaluating resident assessments and care plans. Your work ensures that every resident is supported in attaining the highest possible level of functioning while adhering to current state and federal guidelines.
You will serve as a key resource for the interdisciplinary team, coordinating the MDS, CAAS, and care planning processes from admission through ongoing reviews. This role involves a dynamic day-to-day rhythm of interviewing residents and families, observing changes in physical and emotional conditions, and collaborating with physicians to tailor care and treatment plans. Beyond clinical duties, you will act as a mentor to other MDS nurses, develop training on supportive documentation, and contribute to quality improvement initiatives and community surveys.
