We are seeking a dedicated MDS Nurse to join our team at the Village at Victory Lakes in Lindenhurst, IL. In this PRN (as needed) capacity, you will play a pivotal role in organizing, completing, and evaluating resident assessments and care plans. Your work ensures that every resident is supported in attaining the highest possible level of functioning while adhering to current state and federal guidelines.
You will serve as a key resource for the interdisciplinary team, coordinating the MDS, CAAS, and care planning processes from admission through ongoing reviews. This role involves more than just documentation; it requires observing changes in residents' physical and emotional conditions, interviewing residents and families to gather accurate assessments, and consulting with physicians to tailor care and rehabilitation plans. You will also mentor other MDS Nurses, fostering professional growth and expanding the team's knowledge of assessment processes.
As a vital part of our community, you will help maintain a professional atmosphere that respects the dignity and well-being of each resident. Your contributions will directly impact our Quality Improvement and Quality Assurance programs, ensuring we meet the highest standards of care during community surveys and inspections.
