About the Role
This PRN MDS Nurse position is based at the Village at Victory Lakes in Lindenhurst, IL. The role is designed to ensure residents attain the highest possible level of functioning by organizing, completing, and evaluating resident assessments and care plans in strict accordance with state and federal guidelines.
As a key member of the interdisciplinary team, you will coordinate the MDS, CAAS, and care planning process from admission through ongoing reviews. Your work involves establishing Assessment Reference Dates (ARDs), conducting resident and family interviews, and ensuring nursing documentation accurately reflects the care plan. You will also serve as a resource and mentor to other MDS nurses, fostering professional growth while maintaining a professional atmosphere that respects the dignity and well-being of every resident.

Candidates must possess a nursing degree from an accredited institution and a current unrestricted license as an LPN or RN in the practicing state.
