About the Role
This position serves as the operational backbone for the facility's regulatory compliance and reimbursement strategy. The primary mission is to coordinate the Resident Assessment Instrument (RAI) process, ensuring that every resident's assessment is accurate, timely, and reflective of their physical, mental, and psychosocial status. By bridging clinical care with financial requirements, this role guarantees that the facility receives appropriate reimbursement for services provided under Medicare, Medicaid, OBRA, and Managed Care programs.
The Care Management Specialist acts as a vital resource for the interdisciplinary team, facilitating efficient care plan development and identifying opportunities for optimization. You will work closely with the Director of Rehab, Business Office, and Regional Care Management Specialists to manage case mix, track skilled customers, and ensure compliance with complex federal regulations. This role is critical for maintaining data integrity and supporting the facility's financial health through precise documentation and strategic analysis.

- check_circleEnsure accurate and timely completion of all Medicare and Medicaid case-mix documents for appropriate reimbursement
- check_circleCollaborate with the Director of Rehab to determine appropriate assessment reference dates for Medicare assessments
- check_circleTrack Skilled customers utilizing Case Management Tools to determine continued Medicare and Managed Care eligibility
- check_circlePerform concurrent MDS reviews to assure appropriate RUGs category is achieved through clinical information capture
- check_circleParticipate in the interdisciplinary team process to facilitate efficient and effective care plan development
- check_circleEnsure accurate and timely completion of all MDS assessments including PPS, Unscheduled, Admission, Quarterly, and Annual
- check_circleEnsure timely electronic submission of all Minimum Data Sets and secure back-up personnel to complete this process
- check_circleCommunicate with the Business Office Manager and Administrator regarding RUG distribution and reimbursement impact
- check_circleConduct regular audits of the MDS process including validation of coding documentation and utilization of Data Integrity Audit reports
- check_circleEnsure assessments accurately reflect the physical, mental, and psychosocial status of each resident
Requirements
