The LVN Care Coordinator serves as a vital link in our home care delivery system, ensuring that patient care is coordinated, managed, and delivered with the highest standards of quality. This role exists to bridge the gap between clinical needs and service execution, supervising clinical personnel while guiding the development of comprehensive care plans.
In this position, you will manage case referrals by reviewing patient information to determine specific home care needs and assigning the appropriate clinicians. Your day involves evaluating cases through record reviews and conferences, verifying clinical impressions, and providing real-time guidance to clinicians to promote effective performance. You will actively participate in case conferences, conduct quarterly record reviews, and assist in the screening, interviewing, and orientation of new team members.
Beyond clinical oversight, you will contribute to the organization's strategic goals by assisting in the formulation of policies and procedures, supporting continuing education programs, and engaging in community activities that highlight our role in the healthcare delivery system. This role requires a strong customer service orientation and the ability to collaborate effectively with the Clinical Director and organization personnel.
