About the Role
Encompass Health seeks a compassionate LPN/LVN to deliver high-quality nursing care in our inpatient rehabilitation setting. You will support RNs and physicians by monitoring patient status, administering medications, assisting with treatments and ADLs, and reinforcing individualized rehab plans. Collaborating with an interdisciplinary team, you will educate patients and families, ensure accurate documentation, and promote a safe, healing environment aligned with our patient-centered, growth-focused culture.

Key Responsibilities
- check_circleProvide direct patient care under RN and physician direction in an inpatient rehab environment
- check_circleAdminister medications and treatments safely and accurately
- check_circleMonitor, document, and report changes in patient condition
- check_circleAssist patients with mobility, ADLs, and rehabilitation activities
- check_circleParticipate in development and implementation of individualized care and rehab plans
- check_circleMaintain accurate, timely electronic health records and documentation
- check_circleSupport infection control and patient safety protocols
- check_circleEducate patients and families on care, rehab goals, and discharge plans
- check_circleCoordinate with the interdisciplinary rehab team to optimize outcomes
Requirements
- verifiedActive LPN or LVN license in state of practice
- verifiedBLS certification (AHA or equivalent)
