About the Role
Encompass Health seeks a compassionate LPN/LVN to deliver high-quality nursing care within our inpatient rehabilitation setting. This role is vital to our mission of fostering a patient-centered, growth-focused culture where every individual receives a safe and healing environment. You will serve as a key support to RNs and physicians, bridging the gap between medical direction and daily patient needs.
Your day will be defined by direct patient care, where you monitor patient status, administer medications, and assist with activities of daily living (ADLs) and rehabilitation activities. Beyond clinical tasks, you will play an educational role, teaching patients and their families about care plans, rehabilitation goals, and discharge strategies. By collaborating with an interdisciplinary team, you will help implement individualized care plans that optimize patient outcomes while maintaining accurate documentation and upholding strict infection control and safety protocols.

Requirements
