About the Role
As a Licensed Practical Nurse (LPN) in the Waterbury, CT area, you will provide skilled nursing care reflective of our mission to the ill or injured in home, hospital, or extended care facility settings. This role is vital to ensuring continuity of care, working under the direction of the Nurse Case Manager and/or Clinical Manager for home care cases, or under the shift supervisor for institutional assignments.
In this position, you will coordinate care with community resources and designated Registered Nurses, ensuring that adult, geriatric, pediatric, and neonatal clients receive accurate procedures, treatments, and documentation. You will play a key role in promoting education for clients, families, and the community, while tactfully communicating observations and concerns to the care team. Your ability to recognize the unique cultural diversity, needs, and physical conditions of our patients will be essential in fostering a supportive environment.
