About the Role

As a full-time day-shift Licensed Practical Nurse (LPN), you will serve as a vital link in our collaborative care network. Working alongside RNs, providers, care coordinators, and support staff, you will deliver comprehensive clinical care that addresses both medical needs and social determinants of health. Your day will involve direct patient care, from obtaining vital signs and administering medications to performing wound care and monitoring patient conditions. Beyond clinical duties, you will play a key role in patient education, offering culturally sensitive guidance on treatment plans, chronic disease management, and wellness to a diverse population.

This position is more than just clinical execution; it is about advocacy and connection. You will help bridge gaps by connecting patients to appropriate resources and participating in quality improvement initiatives and community-focused health programs.
