About the Role
As a Licensed Vocational Nurse, you will serve as a vital link in our interdisciplinary team, delivering skilled nursing services directly within patients' homes. This role exists to ensure that patients receive safe, compliant, and personalized care that supports their journey toward independence. You will administer medications, perform comprehensive assessments, and manage wound care while collaborating closely with supervising RNs, physicians, and the broader care team to monitor progress and optimize outcomes.
Your day will involve traveling to various patient locations to provide hands-on care, educate patients and families on disease processes and safety precautions, and maintain accurate documentation in electronic medical record systems. By practicing infection prevention and emergency procedures, you will help create a safe environment for every visit, ensuring that clinical standards are met while building trusting relationships with the communities you serve.

Requirements
- verifiedActive Licensed Vocational Nurse (LVN) license in the state of practice
