As a Licensed Practical Nurse (LPN) specializing in Behavioral Health for Home Visits, you will join an elite team of healthcare professionals dedicated to making a real difference in patients' lives. Your work will focus on keeping patients home where they want to be, supporting their journey of health wherever they call home.
In this PRN weekend role based in the Hartford area, you will operate without traditional shift work, instead focusing on field runs that offer excellent compensation. You will verify plans of care, implement patient safety strategies, and collaborate closely with RN Case Managers and physicians to prevent avoidable hospitalizations. This position is designed for those with a true calling to develop compassionate care and deliver specialized nursing skills directly in the community.
