About the Role

The Licensed Practical Nurse (LPN) serves as a vital link in our care continuum, providing direct nursing support to residents across short-term rehabilitation, long-term care, and memory care units. In this role, you will partner with Registered Nurses, physicians, therapists, and caregivers to implement individualized care plans that address the unique needs of each resident.

Your day-to-day involves administering medications and treatments, monitoring vital signs and resident status, and maintaining accurate documentation. Beyond clinical duties, you will play a key role in educating residents and their families regarding care plans and health needs, ensuring smooth transitions during admissions, discharges, and transfers.
